The Portal is a communication tool offered to patients as a convenience for viewing select portions of your health record and submit comments that may become part of your medical record. You will be able to receive messages and reminders from your therapist about test results, exercises and appointments. Your therapist may decide what types of information to release to your Portal. All information will be shared over a secure, encrypted connection.
You must be 13 or older to enroll to use the Portal and when you do, you will create a password that allows you to login when you want to use the Portal. Parents, guardians and legal representatives ("Proxies") of patients younger than 13 may enroll for their minor children or create unique passwords for access to their adult children's accounts, with their approval. If your relationship to such person changes, you must inform us immediately.
It is important to keep your access code, ID and password completely confidential. You should change your password immediately if you believe your security may have been compromised.
Do not use the Portal for any information that is not related to your physical therapy treatment, including information about substance abuse, mental health, STDs, HIV/AIDS, genetic testing, pregnancy, or prenatal care.

Your therapist and provider will not be liable for any loss or claims of any kind resulting from your failure to keep your password confidential or to read messages in a timely manner. You agree that all communications, including seeking advice or service through the Portal will be in regard to your health and condition. Asking from advice or service on behalf of another person could be potentially harmful and is a violation of these terms. We do not assume liability for and disclaim any and all liability in connection with personal health information or services for such person.
